Joint Health and Wellbeing Strategy 2026 to 2031

We are inviting residents to share their views on our draft Joint Health and Wellbeing Strategy 2026 to 2031.

We want to work with residents, carers and community groups to shape how health and care services develop and are delivered in Hillingdon over the coming years.

Some changes are part of national NHS plans, including more care being provided in local communities and a new Hillingdon Hospital. How these changes are delivered locally is a choice, and we want your views before plans are finalised.


The challenges we need to address

Hillingdon has many strengths, but there are growing pressures on health and care services.

These include:

  • more people living with long‑term conditions such as diabetes, high blood pressure, asthma and heart disease
  • increasing mental health needs for children, young people and adults
  • some neighbourhoods experiencing poorer health and greater hardship
  • more people using urgent and emergency services
  • an ageing population, with more people living with frailty, dementia and complex needs.

Our shared vision

By 2031, Hillingdon will be a fairer, healthier place where everyone can reach their full potential.

To achieve this, we want to:

  • reduce health gaps between neighbourhoods
  • support people earlier to prevent illness
  • make services easier to use by making them more accessible to people and communities.

What could change for residents?

Over the next few years, you could see:

  • more care provided close to home, rather than travelling to hospital
  • faster help when something goes wrong, especially for older people and people with long‑term conditions
  • better joined‑up care, so services talk to each other, and you don’t have to repeat your story
  • more focus on prevention and early help, not just crisis care
  • clearer support for families and unpaid carers.

What will not change

  • You will still be registered with your GP.
  • Emergency services will still be available when you need them.
  • NHS care will remain free at the point of use.

Our priorities

The strategy focuses on four long‑term priorities:

  1. Best Start in Life: Supporting children and families from pregnancy through childhood.
  2. Live well: Helping adults stay healthy, manage long‑term conditions and support their mental wellbeing.
  3. Age well: Helping older people live independently for as long as possible, with quicker support at home and better recovery after illness.
  4. Healthy places: Creating neighbourhoods that support good health, connection and wellbeing.

How will we know if this is working?

By 2031, we want to see:

  • more children ready for school
  • fewer health gaps between neighbourhoods
  • better management of long‑term conditions
  • improved mental health for all ages
  • fewer avoidable hospital visits
  • more older people living independently at home
  • better support for unpaid carers.
