Your Life Line 24/7

The Your Life Line 24/7 team can help if you or someone you care for is in need of advice, support or nursing care at the end of their life.

The person in need of the service must be 18 years or over, be registered with a Hillingdon GP and or be living Hillingdon.

The team is made up of clinical nurse specialists who offer:

  • a 24/7 phone line for advice, support and guidance
  • an overnight nurse visiting service
  • a point of coordination between all of the services you are accessing.

Your Life Line is open 24 hours a day, 7 days a week.

You can ask your GP or district nurse to make a referral.

CNWL Community Palliative Care Team

Hillingdon Palliative Care Team provides specialist care in community settings for patients with life-limiting illness.

Patients referred to the service may have a malignant or non-malignant disease and disease diagnosis and:

  • are not suitable for active, curative treatment
  • are likely to be in the last months of life
  • have complex needs requiring specialist management.

Michael Sobell Hospice

Michael Sobell Hospice provides care, comfort and support for local people facing life limiting illnesses.

The clinical team offers medical, nursing and emotional care along with practical support for patients and their families.

The hospice provides inpatient and day therapy services. The Inpatient Unit has 10 beds. They also provide a 24-hour consultant-led advice line for the use of health care staff.

Harlington Hospice

The hospice provides hospice at home services, counselling, lymphoedema services, and a day hospice, as well as bereavement and loss therapy support for children and adolescents.

Hillingdon Hospital palliative Care Team

The Palliative Care Department is based at Hillingdon Hospital and has strong links to the community. A team of specialist nurses, doctors and other healthcare professionals provide palliative care and symptom and pain control for patients with cancer and life-limiting illnesses.

Hillingdon Health Care Partners (HHCP)

Hillingdon Health Care Partners (HHCP) aim to provide an integrated system of care across Hillingdon. Improving end of life care is one of the key aims for the service.

The HHCP is made up of Hillingdon Hospitals NHS Foundation Trust, Central and North West London NHS Foundation Trust (CNWL), H4All, a partnership of voluntary sector health care providers; and Hillingdon's newly formed GP Federation, which brings together all of Hillingdon's GPs.
